Stop work order after crane supervisor falls to death from 20m

Penang occupational safety and health department director Hairozie Asri said the employer had been instructed to outline measures to ensure the safety of workers.

BUTTERWORTH: A supervisor died after falling from a height of more than 20m while inspecting maintenance work carried out on a ladle crane at a manufacturing plant here yesterday.

Penang occupational safety and health department (DOSH) director Hairozie Asri said an initial investigation found the victim was a 51-year-old Malaysian who was supervising ladle crane maintenance work carried out by workers at the factory.

“A team of Penang DOSH investigation officers went to the scene to conduct an investigation as soon as they received information about it.”

Hairozie said a stop work notice was issued immediately at the factory site until the investigation is completed.

He said the employer was also instructed to carry out an internal investigation to identify the cause of the incident, outline improvement measures and present a report to DOSH.

”Penang DOSH will carry out a follow-up investigation by recording statements from witnesses and conducting follow-up visits to ensure prevention measures have been taken,” he said. - FMT
